Please read before completing form:

The ceremony will take place Sunday, May 5th, 2024 outside by the crosses.
Please meet in the Front Office/Conference Room at 9:00am for the Water Baptism class.
Please wear dark walking shorts or slacks; no bathing suits or short shorts.
Do not wear white clothing. (Once submerged in water, white clothing becomes see through clothing).
Bring additional clothing attire to change and a towel.
Only those who reserve will be called up. To reserve, please complete the portion below and submit no later than January 24th.
You will receive a call or email to confirm.
A Water Baptism Certificate will be mailed or given to you.
